Distinguished Service Award

Selection Criteria: A nominee must

                  1. Be nominated by a NAHC member cooperative, association of cooperatives,
                      association of cooperatives, or professional member.
                      2. Demonstrate exemplary achievement on behalf of a housing cooperative or
                      association of cooperatives.
                      3. Nominees may be a cooperator, director, agent, employee, professional,
                      public official, or other individual whose work on behalf of the cooperative
                      deserves recognition from the National Association of Housing Cooperatives.
                      4. Nominees need not be present for the award.

         page and attach to application. Include additional materials as requested.
     2. Applications must be postmarked no later than June 30.
     3. Entries, information, and attachments become the property of the National Association of Housing Cooperatives.
     4. Include $250 payable to National Association of Housing Cooperatives.

Nomination

     Question 2.  Describe the impact the efforts of the nominee had on the cooperative or association of cooperatives.
